President Obama declaring today that the health care overhaul upheld last week by the Supreme Court is quote here to this day.

But some states -- already defined parts of the law joining me now Fox News senior medical contributor doctor Marc Siegel.

It's the Medicaid issue doctors all states are incredibly concerned about the cost burden.

It seems that that warm more of them are signing new and how do you think that this is going to be the trend that we're gonna -- -- a fight.

You know they may not actually pay what they're supposed to pay in addition to that there's administrative cost take New York State.

It's one to two billion dollars to administer just to administer the Medicaid expansion over.

Ten years so this is a lot of money and you know what we could end up with a situation where patients.

You know our 133%.

Of the federal poverty line and end up qualifying for the new Medicaid but -- state is not giving it.

Medicaid itself is going bankrupt.

Because what is Medicaid Medicaid is it easy insurance to overused.

I love it when you know supporters of Obama care say wow.

People go to the emergency room you know and and nobody gets paid for what.

They say is it's preventative care doctor -- -- may say that while they're gonna go -- we're preventive care so this stop them down the line from coming down with.

You know stage four cancer can be caught -- 81.

That's that the administration says but you don't think that happen.

I believe in preventive medicine but I believe preventive medicine starts before you get to the doctor's office it has a lot to do with -- lifestyle it's what you eat it's what you way.

-- there when you get to the doctor's office yes I can do things to prevent things but studies have never shown.

The what I do in the doctor's office actually impacts later on that's actually completely subject to debate.

I don't believe -- all the regulations.

And restrictions of Obama care that we're actually gonna save the health care system money.

Because you can't decide those things from on high Cheryl.

Those go on on a one on one basis I could take a single patient and tell you if I -- a colonoscopy on -- patient.

Chances are they won't get colon -- but I'm nervous hearing a federal government talk -- Let me let me -- is that Kaiser Family Foundation cannot but this average price per family policy already risen by 2200 dollars.

During the Obama administration to get worse.

Of course it gets worse because of the things you just talked about.

Of course if you give a comprehensive insurance I -- worried well insurance it's not insurance designed only for people when they get very sick its insurance that you -- making -- any did the doctor will assist that big enormous price.

And -- what about the 250 million Americans already have health insurance.

Do they suffer in the types of service the quality of service that they receive if we move forward as is right now.

Well I think that they will have to in the sense that their doctors are already overworked many -- -- doctors are now moving to hospitals are joining larger groups.

There right hospital right.

Right there where something coming down the -- called -- accountable care organization which is gonna reward.

It's gonna reward when you actually do something of value coal quality care but it's getting harder to prove that in my office it's going to be easy for hospital to prove that -- they'll have a list of specific business manager that sits there are saying.

We can prove that this is a benefit we can prove that this is a benefits everything becomes more and more of a business model.

For a larger business and many hospitals and many large groups -- already for seeing that and moving in that direction.

Private practice is in trouble the private practitioner certainly the solo practitioner.

They become extinct in this climate so when President Obama says you get to keep your doctor.

Well that doctor may not be practicing -- where he is now he may be practicing -- -- hospital in May be practicing in the group he may have retired early you may have changed may be wishing he had become a lawyer.
